Community Health Centres of Northumberland	Port Hope (Municipality of)	Clients and visitors are asked to wear masks in the centre. Provides primary health care, health promotion and community development services. Practitioners include physicians, nurse practitioners, diabetes educators, community dietitian, dentist, dental hygienist, health promoter, counsellor and community outreach worker. Primary Care Outreach - Physicians and Nurse Practitioners assess and treat non-life-threatening injuries or illnesses Geriatric Assessment Intervention Network (GAIN) - A network of coordinated healthcare services and partners working together to strengthen the supports and capacity of frail older adults. Memory Clinic (MINT) - team approach for the early diagnosis, treatment and support of problems associated with memory loss.
