Practitioners include physicians, nurse practitioners, diabetes educators, community dietitian, dentist, dental hygienist, health promoter, counsellor and community outreach worker. - Primary Care Outreach - Physicians and Nurse Practitioners assess and treat non-life-threatening injuries or illnesses
- Geriatric Assessment Intervention Network (GAIN) - A network of coordinated healthcare services and partners working together to strengthen the supports and capacity of frail older adults.
- Memory Clinic (MINT) - team approach for the early diagnosis, treatment and support of problems associated with memory loss.
- Diabetes Education Programs - One-on-one education and supports, and groups for people living with diabetes
